Translation needed

Hello, I am adding a guetsbook section to my site, but since I do not really know any French the translation to French is a bit hard for me.

It starts at guestbook, I found:
-Livre D'or  (Book of Gold??)
-Journal des visiteurs (Too long)

So what would be the right translation??

Also, I have some extra stuff to translate like:
Name -> Nom
Email -> Email
Subject -> Sujet
Message -> Message
Cancel -> Chancel

And some additional lines, can somebody help me with them??

"Livre d'or" is good. It means "Gold book" which is a book that you can sign and tell you apreciation after a spectacle, a dinner, ...

"Cancel" => "Annuler"

Other translations are good.

I have some more lines, basically this is just more or less a translation from parts of this forum.
